What is the continued eligibility questionnaire? ›

If you are on automatic payment, you will receive a Disability Claim Continued Eligibility Questionnaire (DE 2593) after 10 weeks of payment. You must return this form to us to certify that your disability continues. Your benefits will stop if you do not complete and return the DE 2593.

While it's essential to be thorough when discussing your disability, you should avoid mentioning unrelated medical issues or conditions. Only discuss the disabling medical problems directly related to your disability claim. Bringing up minor or unrelated issues may confuse or distract the SSA representative.

When your disability check isn't enough to live on, you may have additional options at your disposal. For example, you may qualify for extra help in specific areas such as health care costs, food, and housing. Different federal, state, and local programs may be available.

You may be paid for periods longer than 52 weeks if your benefits are reduced because you returned to work on a part-time basis, or if you received other money during your disability claim period. You can contact the Social Security Administration if your disability extends beyond what DI allows.

SSDI benefits are intended only for people who are unable to work due to disability. If you participate in what the SSA deems “substantial gainful activity” and earn more than a certain amount of money each month, you could be cut off from benefits.

Contact Your Physician/Practitioner to Submit Your Medical Certification. Provide your Form Receipt Number to your physician/practitioner so they can submit the medical certification. The physician/practitioner must submit the certification no later than 49 days after your disability begins or you may lose benefits.
